Thank you for choosing the Duke Executive Health program. Please complete the online questionnaire as soon as possible. Your responses about your current physical health, medical history, and additional background will be reviewed by an Executive Health Physician. After the review is complete, we will contact you to schedule an appointment.
NOTE: After reviewing your completed questionnaire, an Executive Health Physician might call you prior to your appointment.
Your responses are completely confidential. Answer all sections as completely and honestly as you can.
Completing the Online Questionnaire
The questionnaire captures information about your Medical History, Fitness Screening, Stress Management, Nutritional Profile, and optional tests and screenings. Altogether it should take you approximately one hour to complete.
You can stop at the end of any screen and return later. Just click SAVE MY INFORMATION. You may find it helpful to bookmark the login screen in your browser, so that you can return to it easily. Otherwise you will need to click on the link in the email to get the login button.
Before you begin, have ready:
- Account name and password provided to you
- Name and address of your personal physician
- History of illnesses, surgeries, and injuries (including the year in which they occurred)
- Names and dosages of prescription medications, vitamins, supplements, or pain relievers (such as Tylenol or Advil) that you take regularly
- Allergies to medications (if any)
HINT: We suggest using the copy/paste function to copy the password from the email you will receive, into the login screen. Also, make sure that caps are set to off on your keyboard as the username and password are case sensitive.
Requesting Your Appointment
The last section in the online questionnaire is the Appointment Request Screen. Once you have completed it, click on the SUBMIT button at the bottom of the page. This will transmit the forms to us and then we can proceed with setting up your appointment.
At any time during the process, if you encounter a problem with the forms, you can click on the FEEDBACK button on the left side of the screen or you may call us at 919-660-6606.
Accessing your Patient Portal
We now have the capability to load your lab results as well as the Recomendations from the Fitness and Nutrition Specialists, to your Patient Portal, so you can review them online at your convenience. If you would like to take advantage of this new function, please let us know.
